Provides services to adult mental health. The ACT Case Manager assists in developing, evaluating, and coordinating treatment plans and recovery services. Coordinates and implements the ISP in accordance with the following State rules: a) assist the individual in the development of a recovery plan; b) develop a functional assessment of individual’s strengths and needs toward achieving recovery goals; c) assist the individual in identifying recovery goals and objectives; d) identify services to be provided to achieve the goals and objectives and either provide those services or arrange for their provision as appropriate within the scope of behavioral health services; and, e) assist the individual in identifying natural supports, family or non-behavioral health resources toward achieving the recovery goals.
Bachelor’s degree in a human service related field and/or associate’s degree in a human service related field and two year work experience in behavioral health. Working unconventional hours: weekends/evening/holiday. Must pass a criminal background check; maintain a current driver’s license, current automobile insurance, and maintain a clean driving record. Bilingual English/Spanish preferred.
