As part of a multi-disciplinary team, the Lead Clinician will meet and work with participants in Assisted Outpatient Treatment (AOT) to help them sustain recovery. The Lead Clinician’s primary responsibilities are to engage participants and to coordinate mental and primary health and recovery services that will ensure stability and community integration. This position is responsible for maintaining the highest quality provision of direct individual and group therapeutic and case management services while fostering teamwork among staff and across departments and programs.
AOT lead clinician will work with the courts, hospitals, and other referring entities, and within the ACT program framework to provide services to patients referred to the AOT. AOT (Assisted Outpatient Treatment is a unique evidence-based care approach for persons with a serious mental illness who are repeatedly at risk for psychiatric hospitalization, homelessness, or arrest, and who have been reluctant to participate in ongoing mental health treatment. This model offers an effective and compassionate option for promoting the safety, stability and well-being of those who participate. An essential aspect of La Clinica de Familia’s AOT program is a “whatever it takes” philosophy that empowers and supports participants in creating their own plans and goals for short term recovery and long term wellness, with AOT staff providing very high levels of support and resources to help clients achieve their goals.
A small caseload is shared with a team which includes the ACT team lead, psychiatric prescriber, and the Lead Clinician, who work together to engage potential clients, complete a comprehensive assessment, develop and implement care plans, and access appropriate community resources. Crisis prevention, intervention, stabilization, and recovery are achieved through team-based care strategies that are proactive, adaptive and personalized. The AOT team works collaboratively with the ACT (Assertive Community Treatment) team as well as other adult programs as needed, providing outpatient mental health services, addiction treatment, medication support, and other services to serve patients with complex health and mental health issues.
In addition to having exceptional clinical skills, the lead clinician’s professional experience will reflect warmth, ingenuity, integrity, leadership, resilience, flexibility and, most of all, deep satisfaction working with people living with a serious mental illness. A sense of humor is a must as are strong ethical reasoning and excellent oral and written communication skills. This is a job for someone who is highly motivated to improve outcomes for people with serious mental illness through direct service and effective advocacy with family members and service providers. This position also involve working directly with leaders in public mental health, law enforcement, court systems, and other community agencies establishing the AOT system of care, and maintaining La Clinica de Familia’s reputation for friendly and trustworthy collaboration with such agencies.
Master’s degree from an accredited university, experience working with SMI populations.
Must pass a criminal background check; maintain a current driver’s license, current automobile insurance, and maintain a clean driving record. Bilingual English/Spanish preferred.
