Job Description:

As a Care Coordinator working with our CareLink NM Health Home members, you will be providing a vital service to our members through care coordination activities including conducting assessments, care planning, telephonic and in home visits, coordination of benefits and services, and linking members to community based resources.

Job Requirements:

A valid driver's license, proof of highest level of education obtained. Must be able to pass a criminal background check to work with adults and children. New Mexico Medicaid Experience. A background working with complex medical, long-term care and/or behavioral health populations. Bilingual in English/Spanish.Must be willing to use personal vehicle in the course of employment and maintain a clean driving record. Must submit to LCDF required background check, TB screen and drug testing.
